The ending was kind of expected, but a few of the events leading to the last chapter were such a plot twist.
It's revealed that Islington, the angel Door was looking for, was Mr. Croup and Mr. Vandemar's employer. Islington wanted Door to look for a key that opened the door that imprisoned him, and for her to open the door. In the end, she opened the door using a fake key to transport Islington to a world far away from what he wanted. Croup and Vandemar both fell, too.
Door, Richard, and the marquis passed out after that. ( Between this, how they got here is hazy. ) They soon awoken to the Black Friars, and were given the full explaination as to why they didn't want anyone to take the key. After resting for a while, Richard is allowed to go to London Above, using the powers of the real Key. He pretty much just lives normally, working and such, until he starts wandering around London three weeks in. He draws a door on a wall, which is soon opened by Door. The marquis appears and Richard, supposedly, returns to London Below.
